How do I obtain a copy of my medical records? To obtain a copy of your records, you must complete and sign a release of records and return the release to Medical Records. This form (.pdf) can be mailed or faxed to you. If you need records from a specialist who sees adults, call 405-271-9194 to have a form sent to you. If you need a child's medical records, call 405-271-2374. Processing the request will take a few days. If you need records from more than one OU Physicians office, specify this when completing the form. Only one form will be needed. The records will be mailed from each office. Is there a charge for obtaining a copy of my records? There is no charge for medical records if your OU Physicians doctor refers you to another physician. If you would like to order copies of your medical record, the cost is $1 for the first page and 50 cents for each additional page. Can my spouse or adult child sign for my records? Only with a Power of Attorney. A copy of the Power of Attorney form must be maintained in your medical record. What is the purpose of the HIPAA Privacy Regulations? The purpose of the Privacy Regulations is to set minimum national standards regarding the confidentiality of individuals' protected health information. "Protected health information" is any individually identifiable health information, including billing and demographic information, that is transmitted or maintained in any form or medium. Many insurance plans include co-payments, co-insurance and deductibles that are the patient’s responsibility to pay. Once your insurance has processed a claim, you will be billed for any deductible/co-insurance amounts according to the Explanation of Benefits received from your insurance company. If you have questions regarding how your insurance company processed a claim, please contact your insurance carrier directly. If you have questions concerning your account balance after your insurance has considered your claim, please contact OU Physicians Patient Accounts at 405-271-1500. Please refer to the detailed billing portion of your most current statement. You will find each date of service and charge itemized along with a notation that insurance has been filed. If you have a question regarding how your insurance is being processed, or to update your insurance information, please call OU Physicians Patient Accounts at 405-271-1500. Is my insurance information correct? To review the insurance information we currently have on file for you, please check page two of your statement. For questions regarding your insurance information, or to update your information, call OU Physicians Patient Accounts at 405-271-1500. To ensure correct billing, always present your insurance card at the time services are rendered. Contact your insurance company or refer to your plan manual for details on covered services. Mail the payment stub located on the bottom of page one of your statement along with your personal check, money order or credit card information in the pre-addressed envelope provided in your statement. You can also call an OU Physicians Patient Accounts Representative at 405-271-1500 to make a credit card payment over the phone. Can I make payment arrangements on my account? Yes. Our Patient Accounts Representatives are ready to assist you in setting up a Budget Plan payment arrangement, Credit Card Budget Plan payment arrangement or reviewing your account for possible assistance through our Reduced Liability Program. I have a compliment or complaint about my experience at OU Physicians, who can I talk to? Please call an OU Physicians Patient Accounts Representative at OU Medical Center and OU Physicians are two separate entities and maintain separate billing systems. OU Medical Center bills for the facility/hospital services; OU Physicians bills for the doctors. For assistance with a hospital bill, call 405-271-4225. Due to the federal privacy standards (referred to as HIPAA) we only release information to the patient or the guarantor listed on the account. Representatives will ask questions of each caller to ensure they are disclosing information to the appropriate person. Any information requested by a third party (such as for medical records) must be accompanied by a recent, HIPAA compliant release form signed and dated by the patient or patient’s legal representative. |
