Obtaining Copies of your Medical Records
If you are interested in obtaining a copy of your health Information, please download, print and complete the Authorizaton for Release of Protected Health Information form linked below. Please check as many items needed and be sure to include encounter dates.

Upon completion, you may fax, mail, or personally deliver your Authorization to the Health Information Management Department at OU Medical Center. Please note, to adequately verify your information, we will need a valid I.D. photo (e.g. drivers license, military I.D. or state I.D.) and a telephone number.
Note to all adult patients of OU Medical Center. On Jan. 11, 2010, the location to obtain a copy of medical records changed. All requests for release of medical records for adults and children will be processed in The Children's Hospital's Medical Records Department, first floor room #1002. The street address is 1200 Everett Drive, Oklahoma City.
You may park in Valet Parking at the east side main entrance of The Children's Hospital at OU Medical Center. Enter through the main entrance and Medical Records Department will be located in the hallway on the left (south) side of the elevator lobby.
